Thomas Pham MD
Internal Medicine & Pediatrics
Dr. Pham was raised in Wichita, KS after immigrating to the States at 4 years of age. Growing up, his parents taught him the value of hard work and a good education. Dr. Pham went to the University of Kansas for undergraduate and medical school. Then he hopped the Kansas-Missouri state line to do his Med/Peds residency at the University of Missouri – Kansas City. After graduating from residency, he joined the faculty and went on to publish research, present at international meetings, and chair residency committees. Dr. Pham came to Denver in 2014 for a change in his medical practice; and of course, to experience the great outdoors. He created Pinnacle in 2018.
Dr. Pham lives in Denver. He enjoys reading, tech, outdoor activities like camping, hiking, and skiing. He also tries to stay fit by training for triathlons.

Stephanie Swanson FNP-C
Stephanie Swanson, FNP-C, is a nurse practitioner who was born and raised in Colorado. She has been a registered nurse (RN) at Denver Health since 2008, working in various departments such as Medical-Surgical (Med-Surg), ACUTE (Eating Disorders), COVID unit, and Cardiology. Stephanie received her Master of Science in Nursing - Family Nurse Practitioner (MSN-FNP) at Regis University and graduated with honors. She worked as a Family Nurse Practitioner (FNP) doing Physical Medicine and Rehabilitation (PM&R) and received a Physiatry Certification which supported her love for physical activity and her belief that we can always improve physically and mentally. Stephanie is passionate about helping people feel their best and is excited to be part of Pinnacle, where she sees patients of all ages.
Stephanie lives in Thornton with her husband, two daughters, and three dogs. In her free time, she enjoys reading, exercising, gardening, and spending time with friends.

Teresa Salazar FNP-C
Teresa is originally from Oklahoma, but has lived in Colorado for the past 7 years. Before that, she lived briefly in North Carolina where she completed her undergraduate studies at the University of North Carolina at Wilmington. She completed her graduate studies at the University of Colorado in Colorado Springs. During her studies, she took a particular interest in pediatric vaccines and vaccine hesitancy and completed research and is published in the Journal of Pediatric Nursing. Teresa started her career as a RN in 2013 and worked in various settings including the ER, orthopedics, surgery, and a COVID unit. Prior to joining Pinnacle, Teresa worked for 2 years as a family nurse practitioner at a small practice in Englewood.
Teresa lives in Denver with her husband, two boys, as well as a dog and two cats. She enjoys cooking, baking, and reading. She stays active by running and hiking.

Carli Luevano MA
Carli was born and raised here in Colorado. She has been a medical assistant since 2015. She began her career working at another Pediatrics and Internal Medicine clinic after graduating from PIMA Medical Institute as an honor graduate. Before then, she was a daycare assistant and taught preschool for three and a half years. While she absolutely loves working with children, she enjoys helping people of all ages, being compassionate and using her skills to help others. She is excited to be a part of Pinnacle Pediatrics & Internal Medicine and to continue giving excellent patient care.
Carli lives in Brighton with her daughter. She enjoys cooking, shopping, walks with her daughter and spending time with family.
